EpiPen® Forms
Students who require EpiPens should have two of them when they are at school. One should be carried with them at all times in a pouch around their waist and another should be in the office. Students who do not have their EpiPens will be sent home for the day, as a reminder of the importance of having this life saving item with them. Please also remember to have the completed forms for the first day of school as well.
Medication Authorization Form
Students who require medication on an ongoing or temporary basis during the school day will need to bring the completed medication form and their medication to the office for storage. No medication (prescription or off the shelf – including allergy or headache), vitamins or any such products should be brought to school unless it is accompanied by the medication form and stored in the office.
